Palliative Care

Hope Palliative Services is a group of professionals that believes that everyone living with chronic, life limiting illness deserves relief from pain and other symptoms, ongoing support, and education.

Working together with the patient's physician, a nurse practitioner and other members of the palliative team provide specialized care and extra support needed to face the daily challenges of serious chronic illness.

Who benefits from Hope Palliative Services?

  • Individuals experiencing pain, anxiety, fatigue, depression, shortness of breath, or other unrelieved symptoms, including the stress of dealing with the disease
  • Individuals with conditions such as cancer, chronic heart or lung disease, liver disease, chronic renal failure, degenerative neurological diseases, HIV and AIDS related disease

What services are provided by Hope Palliative Services?

  • Special care that focuses on pain management and symptom relief.
  • Monitoring of progress, listening to concerns and answering questions about medications and treatment
  • Medication management to help control pain and symptoms
  • Consultation and education on available resources and support
  • Counseling and support for emotional, spiritual and social concerns
  • Companionship and support services offered by trained volunteers

Who pays for Hope Palliative Services?

Palliative care is covered by Medicare and most private insurance policies. A sliding scale fee is available for private pay patients. Hope Palliative Services also cares for patients who are financially indigent.
