RN Care Manager

Provides comprehensive end of life care/symptom control to patients/families and serves as case manager for all hospice services provided.  Applicants must have a current Texas RN license; at least one year of acute care nursing experience required; 1 yr. hospice/home health experience preferred.

All positions require a criminal history background check, post-offer pre-employment drug screen and medical evaluation, and auto liability insurance.
